What this section covers

Measurement-based care improves outcomes, and the barrier is almost always friction: finding the instrument, scoring it, and remembering the last score. These pages remove the first two. Screeners come with their scoring rules and published interpretation bands, and formulation and planning sheets give the structure that keeps a case coherent across months.

None of these pages ask for identifying client information. Use initials and a case reference; keep identity in your record system, not on a loose printable.

Where to start

Pick one broad-band measure and one problem-specific measure per client and repeat them on a fixed cadence — the trend is the clinically useful part, not any single administration. Pair that with a formulation sheet you revisit at review points rather than writing once and filing.

For risk, use a structured framework every time rather than only when you feel worried. Structure is what makes the documented reasoning defensible.
