Sitting with the Urge

OCD — response prevention without white-knuckling

Intrusive Thoughts Normalizer

Everyone has weird thoughts — OCD is the meaning-making

Separates the intrusive thought from the OCD interpretation. Client circles common intrusive content (harm, sexual, blasphemous, doubt), names what OCD tells them it means, and names what it actually is — a thought, not a message.

First sessions after diagnosis, especially for taboo-content OCD (harm, sexual, scrupulosity). Reduces shame quickly.

Reassurance-Seeking Log

Track the compulsion nobody sees

Reassurance-seeking is often the last compulsion clients recognize. This log catches asking, googling, mental review, and confessing — with a relief rating and a script to give family so they can stop feeding it.

Any OCD subtype. Especially harm, scrupulosity, health anxiety, and ROCD.

Family Accommodation Reduction Plan

The single biggest predictor of poor outcome — and what to do about it

Maps current accommodations, ranks them by ease of reduction, and scripts the compassionate withdrawal. Includes escalation planning so families do not cave under the first spike in distress.

Pediatric OCD, adult OCD with cohabiting family, or any presentation where a partner is answering the same question 30 times a day.

Scrupulosity Worksheet

Religious and moral OCD — the fear that looks like devotion

Distinguishes faith from compulsion by function, not content. Names common scrupulous obsessions and compulsions, and picks one compulsion to resist this week. Best done with clergy consultation.

Religious clients with confession loops, prayer redos, or moral scrupulosity. Also works for secular moral OCD.

Harm OCD Worksheet

Intrusive thoughts of hurting people you love

Normalizes harm intrusions, catalogs the avoidance and mental checking that maintain them, and rehearses the 'maybe, maybe not — moving on' response. Includes a distress-wave tracker for after the compulsion is refused.

New parents, caregivers, or anyone terrified they might act on a thought. Requires assessment to rule out true violence risk.

Relationship OCD (ROCD) Worksheet

Chronic doubt about the relationship — and the checking that follows

Names ROCD doubts (do I love them, are they the one, am I attracted enough) and the compulsions that maintain them (checking feelings, comparing, testing attraction). Commits to values-based action while the doubt sits in the passenger seat.

Clients with recurrent, distressing relationship doubt that persists across partners or with a partner they otherwise value.

Contamination OCD Exposure Planner

Graded exposures with response prevention built in

Names the feared contaminant, feared consequence, and current rituals. Builds an exposure ladder with a response-prevention column so exposures are never done without RP. Includes a challenge log for SUDS start / peak / end.

Contamination OCD, health-anxiety-driven washing, or COVID-onset contamination presentations. Requires ERP training.

Just-Right / Symmetry OCD Worksheet

Redoing, realigning, retyping — until it clicks

For OCD driven by a sensory 'wrongness' rather than a feared outcome. Catalogs where 'not right' shows up, plans deliberate-imperfection exposures, and picks one thing to leave crooked for a week.

Symmetry, ordering, counting, or Tourettic OCD presentations. Also useful for co-occurring autism where sensory just-right is prominent.

Pure-O Mental Compulsions Tracker

There are always compulsions — they're just inside your head

Makes invisible mental rituals — review, silent prayer, mental checking, self-reassurance, neutralizing — visible so they can be resisted. Daily log with trigger, mental compulsion, duration, and the response-prevention move.

Clients who report 'thoughts only' OCD, or who plateau in ERP because the ritual moved from behavior to head.

Relapse Prevention

OCD Relapse Prevention Plan

A flare is data, not failure — what to do in the first 48 hours

Captures the client's obsession themes, warning signs, life stressors that historically trigger flares, and the tools that have worked. Books the first-48-hours plan so a bad week doesn't become a lost month.

Discharge planning, end of ERP course, medication changes, or seasonal / life-transition risk periods.

What these OCD worksheets cover

Exposure and response prevention is the treatment with the strongest evidence base for OCD, and it depends on precision: a hierarchy the client helped build, exposures logged honestly, and compulsions named down to the covert ones. These sheets carry that structure between sessions.

Alongside ERP mechanics, the library includes psychoeducation on the maintenance cycle, inference-based and acceptance-flavoured sheets for clients who get stuck arguing with content, and family accommodation plans for households organised around a client's reassurance.

Where to start

Run psychoeducation and a complete ritual inventory first. Build the hierarchy from that inventory, not from the presenting complaint. Then log every exposure — including the ones that went badly — because the pattern across attempts is what shifts belief, not any single trial.

If a client cannot resist rituals at all, drop the exposure difficulty rather than the response prevention. Exposure with rituals intact teaches the ritual.

Frequently asked questions

What are OCD worksheets used for?+

OCD worksheets structure exposure and response prevention: building a graded hierarchy, logging exposures with SUDS at start, peak, and end, tracking rituals the client resisted, and mapping the obsession-anxiety-compulsion-relief cycle so the client can see how relief maintains the loop.

Which OCD worksheet should I start with?+

Start with the OCD Cycle handout for psychoeducation, then a full ritual and avoidance inventory before any hierarchy. The inventory is where covert compulsions — reassurance seeking, mental reviewing, checking memory — usually surface, and ERP fails when those are missed.

Are these OCD worksheets free?+

Yes. Every sheet here downloads as a printable PDF at no cost, and can be sent as a secure client link from a TherapistAssist account.
