Autism worksheets that treat the profile, not the deficit.
Neurodiversity-affirming clinical tools for autistic adults and teens: masking and unmasking work, autistic burnout recovery, sensory diet, stimming logs, interoception rebuilding, social battery mapping, and late-diagnosis integration.
5-4-3-2-1 Grounding
Sensory grounding for panic, flashback, or dissociation
Five things you see, four you feel, three you hear, two you smell, one you taste. Pulls attention back into the room and the body.
Window of Tolerance
Map your nervous system zones — and what brings you back
Hyperarousal, window, hypoarousal. A trauma-informed visual that gives clients language for what their body is doing.
Sensory Regulation Worksheet
Map your sensory profile so you can dose your environment on purpose
An OT-informed sensory profile: identifies overstimulating and under-stimulating inputs, catalogs regulating inputs across six sensory channels, and builds a daily sensory budget to prevent meltdowns and shutdowns.
Autism Masking & Unmasking Worksheet
Name the mask, track the cost, pick one place to unmask on purpose
A structured page for autistic adults and teens: catalogs eight common masking behaviors, quantifies the cost (fatigue, sensory overload, loss of self), and picks one low-stakes context to unmask this week. Framed as dosage, not a switch.
Autistic Burnout Recovery Plan
Recovery is subtraction, not effort
Distinguishes autistic burnout from depression and typical burnout. Guides clients to subtract (social, sensory, task load) rather than push through, with a menu of low-demand restorative inputs and one structural change to prevent relapse.
Stimming Log & Function Worksheet
Stimming is regulation, not a symptom — map function, protect safe stims
A neuroaffirming stimming log: catalogs common stims, sorts them by regulatory function (calm down, wake up, focus, express joy), and — only where a stim is causing harm — offers a same-function safer swap. Explicitly rejects blanket suppression.
Neuroception Log
The below-conscious safety scan
Track when neuroception fires and whether the read (safe / danger / life-threat) matched actual context. Faulty neuroception is retrainable.
Transition Tolerance
Bridges instead of willpower
Log of daily transitions with difficulty rating and specific bridges — 5-min warning, music, movement — because transitions cost more than tasks.
Decision Fatigue Plan
Pre-decide the small ones
Pre-decision menu — breakfasts, workout times, grocery template, capsule wardrobe — so willpower is available for what matters.
ADHD Shame Cycle
Miss → shame → avoid → miss more
Mapping the ADHD shame loop and its intervention points — self-compassionate response, repair action, addressing underlying ADHD needs.
Late Diagnosis Integration
Grief is part of integration
Structured page for the grief of late ADHD diagnosis — the years of shame, what you now understand, what you'd say to younger self, what's next.
Executive Function Energy Budget
Track energy, not hours
Weekly AM/PM energy tracker with drain and refill notes — reveals patterns invisible to a calendar.
Social Battery Tracker
Masking costs more than the calendar shows
Track battery cost and recovery time per event — separate from calendar-time.
Adult Sensory Diet
Daily regulation menu
Regulating vs dysregulating inputs, and a menu for morning / workday / evening that protects the nervous system.
Affirming, not corrective
The worksheets here operate from the neurodiversity-affirming stance: autism is a profile, not a problem to fix. The clinical work is regulation, self-recognition, and building a life that matches the sensory and social architecture the client actually has — not one they've been masking through.
Masking is treated as an adaptation that costs energy, not a flaw. Stimming is treated as regulation, not behavior to extinguish. Sensory profiles are mapped and honored. Social battery is planned around, not moralized about.
A typical sequence
For newly diagnosed or late-identified adults, start with the Late Diagnosis Integration worksheet and the Masking & Unmasking worksheet — those two often reframe the client's entire history in one session. Add the Autistic Burnout Recovery Plan when the presentation is exhaustion, shutdown, or loss of previously-available skills.
For daily regulation, use the Adult Sensory Diet, Stimming Log, and Sensory Regulation Worksheet. For interoceptive gaps, the Neuroception Log rebuilds the internal signal set. For pacing, the Social Battery and Energy Budget worksheets externalize what neurotypical models under-weight. Transitions, decision fatigue, and RSD-adjacent shame cycles are shared with ADHD tooling and pair cleanly for AuDHD clients.
