Hypervigilance De-escalation Ladder

Match the intervention to the intensity

TherapistAssist logo

Hypervigilance is not a thought to argue with — it is a body running a threat scan. This ladder maps the intensity levels and the specific intervention that works at each one, because what helps at a 3 does nothing at a 9.

Level
What I notice in my body
What I notice in my thinking
What actually helps here
Level definitions
1-2 · Baseline alert

Scanning without distress. Orienting, slow exhale, naming the room.

3-5 · Activated

Startle, jaw, shoulders, over-reading tone. Longer exhale, movement, cold water, orienting out the window.

6-8 · High alarm

Racing, certainty of threat, cannot sit. Big movement first, then breath. Reasoning comes last.

9-10 · Flooded

No language, no perspective. Safety only: leave the situation, one anchoring sensation, a known person.
