What is fear of hell anxiety?

Fear of hell anxiety is intrusive, often panicky fear of damnation, punishment, the rapture or being unforgiven, commonly after fear-based religious teaching. It is kept going by safety behaviours such as repeated prayer, confession and reassurance-seeking, which bring short relief but strengthen the fear.

Fear of Hell & Religious Anxiety Worksheet

Map the cycle, don't argue with it

TherapistAssist logo

Intrusive fears about hell, the rapture, damnation, or being "unforgiven" are common after fear-based religious teaching — and can overlap with religious OCD (scrupulosity). This sheet maps the cycle so it can be worked with, not argued with.

Relief lasts?
Safety behaviours I notice
  • Repeated praying, confessing, or re-saying a prayer 'correctly'
  • Seeking reassurance from people, websites, or scripture
  • Avoiding churches, music, words, or news that trigger fear
  • Mentally reviewing whether I'm saved or forgiven
A grounding response for next time
Feet, breath, five things I can see — then return to what I was doing

When to get specialist help

If fears take over an hour a day or rituals feel impossible to stop, ask about Exposure and Response Prevention (ERP) for scrupulosity. Arguing with the fear usually feeds it.

About this worksheet

Arguing with hell anxiety rarely works: every reassurance teaches the brain the threat is real. This worksheet maps the cycle — trigger, fear thought, body, safety behaviour and how long relief lasts — so client and therapist can see the pattern. It lists common safety behaviours and builds a grounding response. It flags when to seek Exposure and Response Prevention (ERP) for religious OCD.

When to use it

  • Hell, rapture or damnation fears after religious teaching.
  • Follow-up to a scrupulosity screen.
  • Panic attacks triggered by religious content.
  • Use ERP with a trained clinician for clear OCD.

How to use it

  1. 1
    Log the cycle

    Several episodes over a week.

Notice what you do to feel safe.

  3. 3
    Practise grounding

    Return to the present.

  4. 4

    Decide on ERP if needed.

Frequently asked questions

How do I stop being scared of hell?+

Fear of hell is usually maintained by reassurance-seeking and avoidance. Noticing the cycle, grounding, and gradually reducing rituals — often with ERP or CBT — help more than trying to prove you're safe.

Is fear of hell a symptom of religious trauma?+

It's one of the most commonly reported effects of fear-based religious teaching, especially when learned in childhood.

Is this religious OCD?+

Possibly, if fears are intrusive and come with compulsions such as repeated prayer or confession. A clinician can assess for scrupulosity.
