Post-Discharge Meal Support Plan (30 / 60 / 90)

The first 90 days — planned before the first hard week

TherapistAssist logo

The first 90 days after discharge from residential or PHP is when relapse risk is highest. This plan concretely covers who's at the table, what the meal plan is, when weigh-ins happen, and what the step-back-up criteria are — before the first hard week hits.

The discharge plan is the treatment

Most step-back-ups are prevented by the plan a client leaves with, not by heroic effort later. Write it before you need it. Share it with everyone who's at the table.
