About this worksheet

The core move of Imagery Rehearsal Therapy (IRT) — the evidence-based treatment for chronic trauma nightmares. The client rewrites the nightmare while awake and rehearses the new version daily. Over weeks, the dream begins to follow the rehearsal. The worksheet has six sections: a short outline of the recurring nightmare (no need to relive it fully — an outline is enough); the point in the nightmare where the client wants it to change; the new version, in which anything is allowed (ally arrives, exit appears, client has power, situation goes absurd, threat becomes safe); how the new version ends; when the client will rehearse this (same time daily, 5–10 minutes, while awake); and a week's tracking of which version showed up on which nights. IRT has strong evidence in PTSD nightmare research and works alongside trauma-focused therapies.

How to use it

    Outline, not full narrative

    Re-narrating the whole nightmare re-activates the trauma. The outline is enough for rescripting.

    Anything is allowed in the new version

    IRT does not require realism. Absurdity, magic, and impossible allies all work — the dream is not obligated to obey physics.

    Daily rehearsal is the mechanism

    The rescript works because it is rehearsed while awake. Without daily rehearsal, the intervention is not IRT.

    Track which version arrives

    The dream shifts across weeks — sometimes the old, sometimes the new, sometimes a blend. Tracking makes the shift visible.

Frequently asked questions

How long until the nightmare changes?+

Some clients report change within a week; most within four weeks of daily rehearsal. Consistency matters more than intensity.

Is IRT safe if the client isn't in trauma therapy yet?+

IRT is one of the safer trauma interventions for standalone use — it works with the dream, not with the memory directly. Coordinate with the treating clinician when possible.
