About this worksheet

The full-form CBT cognitive restructuring worksheet on one printable page. Sections in the order Beck-tradition CBT protocols use them: situation (facts only, no interpretation); emotion and 0–100 intensity; the hottest automatic thought; evidence for the thought; evidence against the thought; the distortion at play (catastrophizing, mind-reading, all-or-nothing, personalizing, should-statement, and so on); the balanced thought that a fair witness could sign; and the re-rated emotion intensity. Cognitive restructuring is not positive thinking — it is testing thoughts against evidence and arriving at a more accurate, usually less distressing, alternative. The worksheet works best when the automatic thought is written as it actually sounded in the client's mind, not translated into cleaner language.

When to use it

  • Depression — the foundational CBT intervention.
  • Anxiety and panic — testing catastrophic predictions.
  • OCD adjunct alongside ERP.
  • PTSD adjunct alongside trauma-focused work.
  • Any presentation where thoughts distorted by emotion are maintaining suffering.

How to use it

    Write the automatic thought exactly as it sounded

    'I'm such an idiot' does the work; 'I am experiencing self-critical cognition' does not.

    Evidence must be observable

    'People definitely hate me' is not evidence — it is another thought. What did someone actually say or do?

    The balanced thought is not a positive thought

    The client should be able to believe it. If they can't, it is not balanced yet.

    Re-rate the emotion, not the thought

    The success measure is whether the emotion softened, not whether the client is now cheerful.

How long does one worksheet take?+

5–15 minutes in session; 10–20 minutes as homework. The evidence columns are what take time.

What if the balanced thought does not shift the mood?+

Usually means the true hot thought was not caught. Use the downward arrow technique to find it, then restructure that one.
