About this worksheet

DBT's PLEASE (sometimes ABC PLEASE) is the physical base emotion regulation depends on. Physical illness treatment, Eating balanced, Avoiding mood-altering substances, Sleep, Exercise. The worksheet audits each row — current state (honest, not aspirational) and a 0–10 rating — and closes by picking the one row to raise by one notch this week. The clinical logic is stubborn: emotion regulation skills work poorly on top of a starved physical base. Sleep deprivation, dehydration, cannabis-blunted affect, chronic pain — each degrades the return on skill practice. The worksheet is not a lifestyle intervention. It is triage for which rung of the base is limiting everything else built on top of it.

When to use it

  • DBT skills training — before or alongside emotion-regulation modules.
  • Mood disorders — depression, bipolar spectrum, PMDD.
  • ADHD executive function work.
  • Recovery — where substance is the A that changes.
  • Any client whose skill practice is not delivering — check the base first.

How to use it

  1. 1
    Honest ratings, not aspirational

The whole tool depends on the current-state column being accurate. A '7 on sleep' when the client is averaging four hours is not workable data.

  2. 2
Raise one row, not all five

Raising one row by one notch for a week produces more downstream regulation gain than a full lifestyle overhaul.

  3. 3
    Substances section is not confessional

Frame as data, not judgment. Cannabis, alcohol, and non-prescribed use all belong; shame in the entry ends the practice.

  4. 4
    Re-audit weekly

The bottleneck row changes across weeks — sleep this month, eating next month. The re-audit surfaces the moving target.

Frequently asked questions

Isn't this just lifestyle advice?+

PLEASE is diagnostic, not aspirational. It identifies the specific physical rung currently limiting emotion regulation returns — that is different from generic 'take care of yourself'.

How is this different from ABC PLEASE?+

ABC (Accumulate positives, Build mastery, Cope ahead) is a wider set that includes PLEASE. Use the ABC PLEASE tracker for the full weekly grid; this worksheet is the deeper audit of PLEASE alone.
