Gender dysphoria coping works best when strategies are built before a wave hits. A coping menu prepared in a calm moment — covering body comfort, presentation, environment, and nervous-system settling — is far more usable than anything invented mid-wave.

Dysphoria comes in waves, and the middle of a wave is the worst time to invent coping strategies. This menu builds yours in advance — what helps your body, your presentation, your environment, and your nervous system — so it's there when you need it.
What my dysphoria usually attaches to (body, voice, being seen, clothes, names, mirrors, photos)
Early signs a wave is starting — how I know before it peaks
Body comfort: things that help me feel at home in my body (movement, fabric, posture, temperature)
Presentation: small things that shift how I feel (clothes, hair, accessories, none — all valid)
Environment: places, people, and media that make it louder — and quieter
Nervous system: what settles me when it's peaking (breathing, cold water, grounding, movement)
Words or actions from others that help in the moment — and ones that make it worse
My 'wave plan': the first three things I'll do next time it hits
How much does dysphoria limit my daily life right now? (0 = rarely, 10 = most days)

Dysphoria comes in waves, and the middle of a wave is the worst time to improvise. This worksheet has the client build their coping menu in advance: what their dysphoria usually attaches to (body, voice, being seen, mirrors, photos), the early signs a wave is starting, and four categories of response — body comfort, presentation shifts, environment changes, and nervous-system settling. It also captures what helps and hurts from other people in the moment, so partners and therapists know the difference. The sheet closes with a three-step wave plan and a longer-term changes section that points toward gender-affirming care conversations without presuming any medical path. Aligned with WPATH Standards of Care v8 and APA guidelines for practice with transgender and gender nonconforming people.
When to use it
Trans and non-binary clients with recurring dysphoria waves.
Between-session coping work alongside gender-affirming care.
Clients early in transition whose dysphoria spikes as awareness grows.
Partners and support people who want to know what actually helps in the moment.
How to use it
1
Build it in a calm week
The menu only works if it exists before it's needed. Schedule it between waves, not during one.
2
Name the early signs
Catching a wave at a 3 is far easier than at an 8. The early-warning list is the highest-value section.
3
Share the 'what helps from others' section
Partners and therapists often default to exactly the response that makes it worse. This section corrects that in the client's own words.
4
Keep the wave plan to three steps
More than three won't be remembered mid-wave. First three things, in order, written down.
Frequently asked questions
Is this a treatment for gender dysphoria?+
No — it's a coping tool for day-to-day waves. Dysphoria that significantly limits life deserves gender-affirming care with a qualified clinician; this sheet supports that work, it doesn't replace it.
Can partners or family use this?+
The 'words and actions from others' section is designed to be shared. It tells the people around the client what helps and what makes things worse, in the client's own words.
What if nothing on the menu works during a wave?+
That's information for the next calm week — the menu gets revised, not abandoned. Persistent severe dysphoria warrants more frequent clinical support.
