First ACT session — creative hopelessness and values orientation
Make room for what hurts, get clear on what matters, and start doing what matters even when it hurts.
Framing
ACT's first session is less about gathering history and more about a felt-sense shift — the client experiences (not just hears) that the struggle with their experience is part of the problem.
"I'm going to ask you about what's brought you in, and I'll also be paying attention to something a little different — what you've been doing to try to fix or get rid of this, and how that's been going. Not because trying is bad, but because sometimes the tools we use to fight our own minds backfire, and it's worth noticing."
Assessment questions
What have you tried to make this go away, control it, or push it down? List everything.
Why · Begins creative hopelessness — the client hears the inventory themselves.
In the short term, did it work? In the long term, did it work?
Why · Distinguishes workability from righteousness of strategy.
What has this struggle cost you — what have you stopped doing, stopped showing up for?
Why · Surfaces values through what's been lost.
If your pain weren't in the driver's seat, what would you be doing with your life?
Why · First pass at values.
Key moves
Creative hopelessness (gently)
Not 'you're hopeless' — 'the strategy of trying to control your inner experience is.' Let them feel the bind.
Introduce willingness
Offer the alternative — make room for the difficult experience, do the valued thing anyway.
Sketch values territory
Use a few life domains (relationships, work, health, growth) and ask what direction matters in each.
Defuse, just a little
When a hot thought shows up, slow it down — 'notice that you just had the thought I'm broken.' Don't argue with it.
Listen for
- Experiential avoidance patterns — drinking, scrolling, withdrawal, perfectionism
- Fusion with self-stories ("I'm just an anxious person")
- Glimpses of values when the client describes loss
- Reason-giving — clients who explain why they can't, when asked what they want
Closing the session
Reflect the central bind back: "You've worked hard to make this go away, and the work itself has been part of the cost." Offer one small willingness experiment for the week.
Notice every time you try to control or escape an uncomfortable feeling this week. Write down what you did, and whether it served what matters.
Common mistakes
- Rushing to defusion exercises before workability is felt
- Doing values clarification as a worksheet exercise without emotional contact
- Arguing with the client's mind instead of unhooking from it
- Mistaking ACT for 'just accept it' — willingness is in the service of action
