Presenting concern

GAD-7 at intake: 16. Worry across multiple domains (academic performance, parents' health, finances, social standing). Headaches, jaw tension, sleep onset insomnia. Reassurance-seeking from partner up to 20x daily.

History

Anxious as long as they can remember. No prior treatment. Came in after partner ultimatum about reassurance behavior. Mother also chronically anxious; family system modeled worry as caring.

Conceptualization across modalities

CBT (metacognitive)

Positive beliefs about worry ('it keeps me safe') drive activation of worry chains. Negative beliefs about worry ('it's uncontrollable, it's harmful') drive secondary anxiety and avoidance. Reassurance-seeking and mental checking maintain intolerance of uncertainty.

Outcome note
