Generalized anxiety disorder
Graduate student with chronic worry, somatic tension, and a belief that worrying prevents bad outcomes.
Presenting concern
GAD-7 at intake: 16. Worry across multiple domains (academic performance, parents' health, finances, social standing). Headaches, jaw tension, sleep onset insomnia. Reassurance-seeking from partner up to 20x daily.
History
Anxious as long as they can remember. No prior treatment. Came in after partner ultimatum about reassurance behavior. Mother also chronically anxious; family system modeled worry as caring.
- Long-standing pattern (chronic GAD harder to remit)
- Positive metacognitive beliefs about worry ('worrying keeps me prepared')
- Reassurance-seeking reinforcing intolerance of uncertainty
- Sleep disruption maintaining cycle
- High insight
- Motivated by relationship strain
- No comorbid depression
- Strong support system
Conceptualization across modalities
Positive beliefs about worry ('it keeps me safe') drive activation of worry chains. Negative beliefs about worry ('it's uncontrollable, it's harmful') drive secondary anxiety and avoidance. Reassurance-seeking and mental checking maintain intolerance of uncertainty.
- Challenge positive metacognitive beliefs via behavioral experiments
- Detached mindfulness for worry triggers
- Reassurance-seeking response prevention
- Worry postponement scheduling
Fused with thoughts as predictions of fact. Heavy experiential avoidance of uncertainty. Worry serves as escape from present-moment discomfort. Life is being lived in service of risk-management rather than chosen values.
- Defusion from worry thoughts ('I'm having the thought that…')
- Values clarification in the domains being missed
- Willingness to feel uncertainty
- Committed action despite anxiety
Treatment plan
Psychoeducation (1–3)
Worry cycle, metacognition, reassurance trap.
Skills (4–10)
Worry postponement, detached mindfulness, reassurance fast.
Exposure (11–16)
Behavioral experiments on uncertainty; deliberate not-knowing.
Consolidation (17–20)
Values-driven action, relapse plan, taper.
Reassurance-seeking dropped to 2–3x/day by week 8. GAD-7 was 7 at week 16. Sleep normalized last.
