Behavioral Activation Worksheet for Depression (Free PDF)

Behavioral activation is the most evidence-backed intervention for depression that almost nobody does correctly. The reason is usually scope: clinicians ask depressed clients to plan a week. Depressed clients can plan an hour.

Shrink the unit

Start with one block — usually morning. Two activities. One mastery (something with a sense of competence), one pleasure (something that used to feel good). Rate each 0–10 before and after. That's the whole worksheet for week one.

Week two: add a second block. Week three: a third. By week four, most clients are running a full day. By week eight, a week. The shrinkage is the intervention, not the deficit.

Mastery vs pleasure (and why both)

Mastery without pleasure produces grim functioning. Pleasure without mastery produces avoidance. The weekly grid forces both into the same view, which is the actual intervention. Depressed clients almost always over-plan mastery (because it feels virtuous) and under-plan pleasure (because nothing feels good). Force the balance.

The "what got in the way" column

Add a third column for blocks the client did not complete. This is not a shame column; it is a clinical data column. Patterns emerge: morning is consistently hard, social activities are consistently skipped, mastery items keep getting downgraded to easier versions. Each pattern is a hypothesis you can test together.

Working with anhedonia

For severely anhedonic clients, the standard "rate pleasure 0–10" returns mostly 1s and 2s for weeks. Reframe: "rate how neutral it felt — how close to not-terrible." Tracking movement from 2 to 4 is the actual progress; waiting for 8s confirms the depression's story that nothing matters.

Common failure modes

  • Planning a full week when the client cannot plan an hour. Always shrink to the unit they can hold.
  • Letting the client substitute "easier" items mid-week. The substitution is itself avoidance. Discuss it.
  • Ignoring the pre/post ratings. They are the entire diagnostic instrument. Without them, the worksheet is a schedule.
  • Treating it as a one-time intervention. BA is a 12-week course of treatment in the research, not a single handout.

Pairing BA with cognitive work

Behavioral activation can stand alone, but it pairs well with thought records once activation produces enough cognitive material to work with. Depressed clients often cannot generate thought records in week one — there is nothing to catch. By week three of BA, the cognitive material starts surfacing.

How long does BA take to work? Most clients show measurable mood improvement in 4–6 weeks of consistent practice.

Is BA appropriate for severe depression? Yes — and the evidence base is particularly strong for moderate-to-severe depression where cognitive work is harder to access.

Should clients keep doing BA after they feel better? Tapering is appropriate once activation is fluid. Most clients keep some lightweight version of the planning indefinitely; the structure is itself protective.
